The default root cause for an incidence of non-conformance raised against a resource (work center) used for a work order operation is displayed if defined in the NCSROORES - Default resource root cause (TC chapter, NCS group) parameter; if this incident was raised against an operational process for a work order the default root cause is defined in the NCSROOPRO - Default process root cause (TC chapter, NCS group) parameter. You can change the default root cause, if required.
If as a result of the root cause analysis more than one cause is determined you are advised to raise additional non-conformances to ensure each root cause is identified. This will ensure each root cause can be corrected or prevented with an Action plan. An example of multiple root causes for a single non-conformance might be that training was inadequate and written instructions were unclear.
